- Ensure program offers individuals served increasing opportunities for employment, social integrations and independent living.
- Participate in the evaluation process, utilizing results to identify needs, establish goals and directions for future planning.
- Attend and oversee the monthly Utilization Review process, including monthly and quarterly data reports to the County and other stakeholders.
- Assist in training staff to complete service plans and risk assessments; oversee service planning and risk assessment for all clients of the program.
- Provide safe, effective and efficient direct care in accordance with established policies, procedures and standards of care; provide services to clients, families and clients' community supports.
- Establish and maintain therapeutic relationships.
- Participate in primary therapy, community meetings, family and group therapy.
- Provide input via reporting observations, concerns and asking appropriate questions.
- Provide leadership in facilitating the team process.
- Ensure all clinical issues are addressed around hospitalization of members served.
- Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.
- May facilitate and participate in team meetings and service planning meetings as assigned.
- Provide 24/7 on-call (cell phone) coverage as needed.
- Master Degree in Social Work or related field and working towards licensure and registered with the Board of Behavioral Science
- 1+ year direct service experience with mentally ill individuals
- Highly preferred: current licensure (LMFT, LMSW, LPCC, etc.)
