RN Care Coordinator

Patient Care Coordinator – Travel RN (Evening Shift)

Location: South San Francisco, CA

Schedule: Full-time, 8-hour shifts (3:00 PM – 11:30 PM)

Contract duration: 13 weeks

Required:

• Active RN California License
• BLS (Basic Life Support) certification (current and valid).
• Minimum 2 years of recent clinical nursing experience (preferably in acute care, case management, or discharge planning).

Preferred:

• Prior experience in case management or patient coordination roles.

Responsibilities

• Coordinate and manage patient care transitions, including discharge planning and follow-up services.
• Collaborate with physicians, nurses, and interdisciplinary care teams to ensure efficient care delivery.
• Evaluate patient needs, identify resources, and advocate for appropriate levels of care.

Pay: $60.00 - $70.00 per hour
