Care Coordinator - CalAIM Enhanced Care Management : Youth Division

We are seeking full time on site staff. An experienced and highly motivated person to join our team as a Care Coodinator for the California Advancing and Innovating Medi-Cal (CalAIM) program. In this crucial role, you will provide outreach and comprehensive case management services to CalAIM recipients in the Enhanced Care Management Program.

Responsibilities:

  • Conduct outreach efforts to identify and engage eligible CalAIM recipients in need of case management services
  • Develop and implement individualized care plans tailored to each recipient's unique needs and goals
  • Coordinate and facilitate access to a wide range of social and community-based services
  • Maintain accurate and up-to-date case records and documentation
  • Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community partners
  • Advocate for recipients to ensure they receive the necessary support and resources
  • Provide education and guidance to recipients and their families on available services and programs
  • Participate in case conferences and team meetings to discuss and review case progress
  • Integrate and assist with clinical operations
  • Perform other duties as needed

Qualifications:

  • Experience working with children in a healthcare, social service, or community setting is required.
  • Bilingual proficiency in English and Spanish is required.
  • Reliable transportation and ability to commute for home visits, outreach, and community meetings is required.
  • Experience in case management is appreciated; training is available for qualified candidates.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • Proficiency in computer applications and electronic medical record (EMR) systems.
  • Strong organizational skills and attention to detail, with the ability to manage multiple priorities and work with a high caseload.
  • Demonstrated ability to work both independently and collaboratively within a team.
  • Compassionate and patient-centered approach to care.
