Care Coordinator - CalAIM Enhanced Care Management : Youth Division
We are seeking full time on site staff. An experienced and highly motivated person to join our team as a Care Coodinator for the California Advancing and Innovating Medi-Cal (CalAIM) program. In this crucial role, you will provide outreach and comprehensive case management services to CalAIM recipients in the Enhanced Care Management Program.
Responsibilities:
- Conduct outreach efforts to identify and engage eligible CalAIM recipients in need of case management services
- Develop and implement individualized care plans tailored to each recipient's unique needs and goals
- Coordinate and facilitate access to a wide range of social and community-based services
- Maintain accurate and up-to-date case records and documentation
- Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community partners
- Advocate for recipients to ensure they receive the necessary support and resources
- Provide education and guidance to recipients and their families on available services and programs
- Participate in case conferences and team meetings to discuss and review case progress
- Integrate and assist with clinical operations
- Perform other duties as needed
Qualifications:
- Experience working with children in a healthcare, social service, or community setting is required.
- Bilingual proficiency in English and Spanish is required.
- Reliable transportation and ability to commute for home visits, outreach, and community meetings is required.
- Experience in case management is appreciated; training is available for qualified candidates.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
- Proficiency in computer applications and electronic medical record (EMR) systems.
- Strong organizational skills and attention to detail, with the ability to manage multiple priorities and work with a high caseload.
- Demonstrated ability to work both independently and collaboratively within a team.
- Compassionate and patient-centered approach to care.
