We are expanding into Lancaster, CA, and are seeking mission-driven Medical Social Workers to join our team under a 1099 agreement.

Compensation:

• Routine Visits: $85 per visit
• Death Visit: $120 per visit
• Out-of-Area Visits: $100 per visit
• Interdisciplinary Group (IDG) Meetings: $45 per meeting
• In-Service Trainings: $45 per session

Responsibilities:

• Conduct psychosocial assessments to determine patients' and families’ emotional, social, and financial needs.
• Provide counseling and emotional support to help patients and families cope with terminal illness, loss, and bereavement.
• Assist with advance directives, end-of-life decision-making, and coordination of community resources.
• Participate in interdisciplinary team meetings and contribute to developing and updating individualized care plans.
• Document services, assessments, and follow-ups accurately and promptly in accordance with regulatory and agency standards.
• Support family conferences, grief support, and discharge planning in collaboration with the hospice care team.

Position Qualifications:

• A graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
• Minimum of one (1) years' experience in healthcare; hospice experience preferred. Must understand hospice philosophy and issues related to death/dying.
• Experience in hospice care preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Possesses and maintains current CPR certification.
• Must be licensed with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
