Role Description

This is a full-time, on-site role based in Raleigh, NC, for a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker-Associate (LCSW-A). The professional will deliver counseling and support services, working with individuals, families, and groups to address mental and behavioral health needs. Daily responsibilities include conducting assessments, creating and managing care plans, providing case management, and collaborating with other healthcare providers to ensure comprehensive support. This role also involves addressing social determinants of health and providing resources to support patient wellness.

Qualifications

• Expertise in Social Work, Medical Social Work, and Case Management
• Experience in Mental Health and Behavioral Health services
• Strong communication, interpersonal, and organizational skills
• Ability to work collaboratively with multidisciplinary teams
• Master’s Degree in Social Work and active LCSW or LCSW-A license in North Carolina
• Knowledge of healthcare systems and social determinants of health
• Experience providing culturally competent care is a plus
