We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to patients and their families, ensuring they receive the necessary care and resources during challenging times.
-Collaborate with medical professionals to assess patients' social and emotional needs
- Develop individualized care plans to address patients' concerns and improve their quality of life
-Proficiency in providing care for geriatric patients and individuals with mental health concerns
-Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare professionals.
Will provide supervision for LCSW hours.
