Now Hiring: Community Health Workers (CHWs) & Youth Care Coaches (YCCs)

Locations: Mendocino County (Ukiah) & Sonoma County

About Synergy Family Services

Synergy Family Services operates across Mendocino, Lake, and Sonoma Counties, offering two primary programs: the Community Health Worker (CHW) Program and the Post-Adoption Wraparound (PAWA) Program.

We are currently hiring for positions in both Sonoma County and Mendocino County (specifically Ukiah).

Our CHW Program serves both community clients and schools. We provide Medi-Cal billable services, including Enhanced Care Management (ECM), to help individuals and families address health, behavioral, and social needs. If selected for a CHW position, you’ll receive an 80-hour state-recognized training that prepares you to work effectively with clients and meet certification requirements.

Our Post-Adoption Wraparound Program (PAWA) supports families with adopted youth who are experiencing emotional, behavioral, or adjustment challenges. Team members in this program are called Youth Care Coaches (YCCs). Many of our CHWs receive additional training and mentorship to transition into YCC roles, expanding their skills and impact as they grow with the organization.

Across both programs, we take a holistic, family-centered approach—partnering with schools, families, and community supports to build lasting systems of care. Each youth and family is surrounded by both natural and professional supports—what we call “wraparound”—to ensure meaningful, sustainable progress.

Responsibilities

  • Provide direct care and support to youth and families in the community, schools, and homes.
  • Implement and follow individualized support or treatment plans tailored to each youth.
  • Support youth with daily living activities, healthy routines, and positive community engagement.
  • Document progress and behavior clearly and accurately.
  • Facilitate group activities and promote positive social interaction among youth.
  • Collaborate with teammates, supervisors, and families to ensure a coordinated, safe, and supportive environment.
  • Help youth maintain a clean and organized space that supports well-being.

Experience & Qualifications

  • High school diploma required; associate degree or higher strongly preferred.
  • Experience working with youth or individuals with behavioral or emotional challenges preferred.
  • Understanding of confidentiality and HIPAA standards.
  • Ability to de-escalate challenging situations and resolve conflicts effectively.
  • Strong relationship-building and knowledge of community resources.
  • First Aid/CPR certification (or willingness to complete training).

Training & Growth

  • Selected candidates will complete an 80-hour CHW training and may receive additional training in the Post-Adoption Wraparound model to become a Youth Care Coach (YCC).
