Clinical Case Manager for Housing, MSW required
POSITION SUMMARY
Clinical Case Managers (CCM) undertake a range of key responsibilities for clients that relate to healthcare, behavioral health, and housing. CCMs will perform bio-psychosocial assessments and create care plans for clients and manage delivery of services with community partners.
Salary is $80-$90,000 annually dependent on experience.

Some Clinical Case Manager Requirements:
- MSW or equivalent required.
- Experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
- Knowledge of principles, ethics and best practices of case management, care coordination, social work, and behavioral health services.
- Experience utilizing client-centered, trauma-informed practices to serve vulnerable populations including individuals experiencing homelessness, chronic medical conditions, and/or behavioral health concerns.
Some Clinical Case Manager Duties:
- Independently manage assigned caseload.
- Provide in-person, community-based case management services that are strengths-based, client-centered and trauma-informed.
- Complete comprehensive biopsychosocial assessment to evaluate needs and create appropriate care plan.
- In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals.
- Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
