Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 2 years of acute care experience required License: LCSW preferred Certifications: BLS, LCSW, MSW Must-Have: Candidates should have experience in case management and discharge planning. Description: The Medical Social Worker is responsible for case management and discharge planning. They will manage a caseload of 20-23 patients. Initial assessments must be completed within 24 hours. Collaboration with the medical team is essential for coordinating discharge plans. Knowledge of various patient diagnoses and demographics is required. Must provide crisis intervention and patient education to support a safe discharge. Experience with the charting system Epic is required. Travel to various facilities may be necessary within the assigned state. A commitment to working three of the five major holidays is expected. Two supervisory references from within the last three years are required. All travelers should be prepared to float between assigned facilities as needed. Candidates must be knowledgeable of community resources relating to the needs of patients and their families. Strong communication and organizational skills are necessary for this role. The position involves working in an interdisciplinary team to ensure quality patient care and efficient service delivery. Candidates must be familiar with documentation standards and care coordination practices. A commitment to ethical practice and adherence to relevant policies and regulations is essential.
