Job Overview
As a Home Health Care Coordinator, you will play a vital role in ensuring that our clients receive the highest quality of care in the comfort of their own homes. You will be the key liaison between clients, families, and healthcare professionals, coordinating services and resources to enhance the well-being of those we serve. Your expertise will help streamline communication and ensure that care plans are effectively implemented, making a meaningful difference in the lives of our clients.
What you’ll do
- Coordinate and manage home health care services for clients, ensuring all needs are met efficiently.
- Communicate regularly with clients, families, and healthcare providers to assess ongoing needs and adjust care plans as necessary.
- Maintain accurate records of client information, including medical histories and treatment plans.
- Assist with scheduling appointments and managing logistics for home visits by healthcare professionals.
- Provide administrative support by handling medical documentation and ensuring compliance with healthcare regulations.
- Educate clients and families about available services, resources, and best practices for health management.
- Collaborate with multidisciplinary teams to optimize client care and outcomes.
- Stay updated on medical terminology and practices to effectively support client needs.
Basic qualifications
- Proven experience in a medical office or healthcare setting is essential.
- LVN License.
- Familiarity with medical terminology to facilitate effective communication within the healthcare team.
Preferred qualifications
- Experience providing medical administrative support or working in a dental office is advantageous.
- Strong organizational skills with the ability to manage multiple tasks simultaneously.
- Excellent interpersonal skills to foster positive relationships with clients and team members.
- Proficiency in office software applications to maintain efficient documentation processes.
