Job Description

We are seeking a detail-oriented and proactive Intake Coordinator to support our Enhanced Care Management operations. In this role, you will play a key part in ensuring seamless onboarding for new members, managing authorizations, and maintaining accurate eligibility tracking. This position is ideal for someone with a background in healthcare administration who thrives in a fast-paced environment focused on patient-centered care.

Key Responsibilities

• Process new member authorizations from health plans and internal referrals, ensuring compliance with ECM guidelines and regulatory requirements.
• Track and monitor member eligibility status, including updates from health plan lists and real-time verifications.
• Manage the end-to-end onboarding workflow for new members enrolled via health plan assignments or field encounters, coordinating with care teams for smooth transitions.
• Collaborate with field staff, providers, and payors to gather necessary documentation and resolve any intake-related issues.
• Maintain accurate records in our integrated care management platform, including symptom monitoring and risk assessment data.
• Assist in early relapse interventions by flagging eligibility changes or authorization needs promptly.
• Support virtual rounding and clinical hand-offs by ensuring new member data is up-to-date and accessible.
• Contribute to quality improvement initiatives by identifying workflow efficiencies in the intake process.

Qualifications

• High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare administration, social work, or a related field preferred.
• 1-2 years of experience in healthcare intake, care coordination, or administrative roles, preferably in behavioral health, mental health, or chronic care management.
• Familiarity with Enhanced Care Management (ECM) programs, Medi-Cal benefits, or similar managed care systems is a plus.
• Strong organizational and multitasking skills with attention to detail.
• Proficiency in electronic health records (EHR) systems and Microsoft Office Suite.
• Excellent communication skills for interacting with members, providers, and health plans.
• Ability to handle sensitive information confidentially in compliance with HIPAA.
• Willingness to work in a team-oriented environment with occasional flexibility for after-hours support.
