Classification Summary

The Human Services of Faribault & Martin Counties has an opening for a full time (40 hours per week) Care Coordinator - Waiver Case Manager. This position is located in Blue Earth, MN (Faribault County)

The HSFMC comprehensive benefit package includes health insurance (including option for high-deductible health plan) with employer sharing in cost, employer-paid $20,000 term life insurance, earned sick and safe time, generous paid leave policy, and 11 paid holidays each year. As a public employer, we provide a defined benefit annuity through PERA (employer contributes 6.5% plus 1%; employee contributes 6.5%). Are you ready to make a difference in the lives of others? We are seeking a dedicated and compassionate MnCHOICES Assessor and Case Manager to join our team. You’ll play a vital role in helping individuals maintain their independence and access the care they need to thrive in their own homes.

In this position, you will conduct MnCHOICES assessments to understand each individual’s strengths, needs, and goals, ensuring they receive personalized care plans tailored to their circumstances. You will work closely with clients and their representatives to develop community support plans, offer guidance on available resources, and provide information about Minnesota Health Care Programs. Additional you will oversee ongoing case management for Home and Community-Based Services (HCBS), monitoring and adapting care plans as necessary to ensure quality and effectiveness. Collaboration is at the heart of this role. You will build strong, professional relationships with clients, caregivers, and partner organizations, all while maintaining high ethical standards.   • Schedules team meetings to review health screenings, new referrals and care coordination activities.
  • Provides appropriate outreach, screening, assessment or advocacy
  • Performs personal follow up contact with client
  • Provides home visits or ongoing short term care coordination
  • Performs outreach contacts to current clients to obtain Health Screenings, provide client education and promotes the use of preventative services to improve client health status related to condition management
  • Provides support at the local level through education of county staff and providers regarding programs and services available and basic operational information.
  • Assists with Quality Improvement Projects and Performance Improvement Projects including outreach activities, data collection and monitoring
  • Provides basic technical support and training to other county staff.

Minimum Qualifications

Merit System Minimum Qualifications
You may qualify as a Social Worker or a Registered Nurse to be put on the Care Coordinator eligible list (see qualifications below).

To qualify as a Social Worker you will need:
  • A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.
or
  • A bachelor's degree from an accredited four-year college or university with a major in any field and one (1) year of experience as a social worker in a public or private social services agency.

OR

To qualify as an RN you will need:
Current licensure as a Registered Nurse and/or PHN certification in Minnesota.

Examination Process

The examination will consist of a rating of training and experience based on the information you provide in your application and your responses to the supplemental questions included in the application process. You must read the questions carefully, answer truthfully, and include any experience claimed in your answers to the supplemental questions in the Work Experience section of your application. Any experience claimed in your answers to the supplemental questions but not included in the Work Experience section of you application will not be credited. You will be asked to verify your degree with official transcripts upon hire.
