Patient Intake Coordinator

A Home Health Service Agency is offering a full time job opportunity of Home Health Care Intake/Coordinator. This position is ideal for a candidate who is active, enthusiastic and has a passion for those who need help.

Title:

Intake Coordinator/Staffing

Responsibilities:

· Processing incoming referrals and admissions, entering the information into system and checking the eligibility of the patient

· Tracking and ensuring that the referral forms are correct and supporting documents are complete

· Scheduling assessments of patients as needed based on clinical presentations and regulations

· Coordinating with nurses, therapists and home aides for new admissions and scheduling necessary patient visits

· Coordinating with physicians, case managers of medical centers to ensure the safety and satisfaction of patients

· Maintaining and tracking accurate, updated records of patients

· Developing, managing and evaluating referral and enrollment reports

· Acting as liaison with family members and referral sources

Requirements:

· Excellent written and verbal communication skills

· At least one year experience of working in home health agency

· Experience with office computer software and home agency system

· (Preferred) Spanish/English bilingual
