Register Nurse (RN) Care Coordinator position will play a critical role in ensuring the quality of care delivered by our caregivers. This position focuses on client assessments, developing and updating non-medical care plans, caregiver training, and compliance oversight in accordance with Florida state regulations.
Responsibilities
- Conduct initial in-home client assessments and develop individualized care plans (non-skilled).
- Review and update care plans as client needs change.
- Provide training, mentorship, and supervision to caregivers (ADLs, safety, infection control, etc.).
- Perform periodic supervisory visits to monitor caregiver performance and client satisfaction.
- Maintain documentation and ensure compliance with agency policies and state requirements.
- Serve as a clinical resource for clients, families, and staff.
- Collaborate with the office team to ensure continuity and quality of care.
Qualifications
- Current Florida Registered Nurse (RN) license in good standing.
- Minimum of 2 years of nursing experience (home care, home health, or geriatric care preferred).
- Strong communication, organizational, and leadership skills.
- Compassionate, patient, and dedicated to supporting clients and caregivers.
- Ability to travel locally for client assessments and supervisory visits.
- Must be able to pass Level II background screening
