Care Manager

***1,000 Sign-On Bonus***

We are a seeking a full-time entry level Care Manager who is responsible for group facilitation, case management, and coordination of recovery planning. May be required to provide some transportation to and from client's residences.

To be considered for this position, you must have at least one of the following qualifications:

• Bachelor's degree in the Human Services field, or
• Associate's degree AND two (2) years' experience in providing Human Services, or
• Five (5) years' of Human Service experience, or
• Possess a relevant professional credential such as Certified Psychiatric Rehabilitation Procedure (CPRP), Bachelor's degree in Social Work, Certified Rehabilitation Counselor (CRC), Bachelor's degree in rehabilitation or Bachelor of Science in Nursing.
• Applicants with one (1) year of experience of pre-vocational / supported employment / job coach experience in addition to the qualifications stated above are preferred.
