Community Home Health Care Services is seeking a compassionate and caring Hospice Social Worker to join our team. Our dynamic culture separates us from our competitors. The focus service area of this position is Berrien County.

Key Responsibilities:

- Provides individual and family counseling, and crisis intervention

- Evaluates coping skills in crisis situations

- Fosters communication between patient and families

- Assists patient and family in acquiring additional community resources, including Medicaid; serves as liaison with social service agencies as necessary

- Participates in program and policy development of social work component

- Documents patient changes appropriately

Position Requirements:

- MSW from an accredited School of Social Work

- 2 years’ experience in a health-related area

- LMSW License, valid driver’s license and vehicle, CPR certification

- Ability to travel within the service area

Job Type: PRN at start

Pay: $35 - $45 / hour
