S:US is seeking a licensed full time Social Worker for our Hill House Program located at 1616 Grand Avenue in the Bronx.

Qualifications

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Under the supervision of the Director of Social Service, the Social Worker is responsible for client case management and the coordination of client services.
  • Work within a crisis driven environment with a knowledge base of residential services for individuals with a history of homelessness, substance use and addiction, IPV, trauma, medical, as well as mental health conditions, and co-occurring disorders.
  • Able to work from the person centered approach, trauma informed care, strengths-based perspective, or solution-focused approach.
  • Has motivational interviewing skills, and knowledge about stages of change.
  • Assist clients with specific stressors associated with daily living, living with certain medical conditions, mental illness or substance use disorders.
  • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being
  • Responsible for the completion of psychosocial assessments of complex clients impacted by multiple systems.
  • Assess client needs and facilitate appropriate linkages for clients to community-based services.
  • Provide education to other members of the team around clients’ psychosocial stressors and needs, to ensure positive outcomes for tenants.
  • Serve as agency/program liaison to community agencies and/or groups
  • Provide clear written reports that capture key findings to include provider areas of strength, and recommendations for improvement.
  • Attend ongoing professional development by the agency, and other funders as needed.
  • Ability to work a flexible schedule when needed which consist of weekend or evening hours
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes
  • Working knowledge of various computer software programs including email, Microsoft Word, Excel, and PowerPoint.
  • Performs other duties as assigned, as deemed necessary by program management.

EDUCATIONAL / TRAINING REQUIRED:

  • A Master of Social Work with at least two (2) years’ experience working with HIV/AIDS, substance abuse, mental illness, and homelessness populations is required.
  • LMSW or LCSW required

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:

  • Experience in assessment, service planning, and community referrals preferred.
  • Knowledge of supported housing, HIV/AIDS and substance abuse is required.
  • Excellent interpersonal, written and oral communication skills.
  • Ability to work as a member of an interdisciplinary team.
  • Bilingual- Spanish/English preferred.
