Birchwood rehab is now hiring a full time social worker for our long term care.

Must have LSW and experience in skilled nursing facility.

Monday through Friday (exempt)

Social Worker, or Case Worker, is responsible for managing multiple clients and their individual needs. Their duties include meeting with clients and their loved ones to discuss recent successes or setbacks to their situations, developing treatment plans for clients with mental illnesses or clients recovering from traumatic events and frequently communicating with clients to ensure their physical and mental health.

The social worker's role should be:
1.Planning for pre-admission and discharge

2.Providing psycho-social assessment and completion of relevant parts of the minimum data set (particularly face sheets,

sections A, AA, AB, AC, E, F, Q and sometimes B) Utilizing the minimum data set to guide the care plan

3.Is aware of the worth and uniqueness of each individual

4.Treats each individual with respect

5. Creates an atmosphere of growth for the individual

6. Adopts a holistic perspective by recognizing the dynamic interplay of social, psychological, physical, and spiritual well-being

7.Provides a physical environment that is supportive rather than challenging or crippling to the individual

8.Fosters a positive self-image for the residents through continued social contact, decision-making opportunities, and

independence

9. Participating (as a member of an interdisciplinary team) in resident care planning

10 Counseling residents and their families Contacting and utilizing community resources on the resident's behalf and serving as

a link between the resident and these resource systems when necessary

11. Advocating for and protecting resident's rights

12. Ensuring the social and emotional needs of each resident are met

13.Stimulating social contact and interaction

14.Promoting the maximum level of independence of each resident
