Master’s Level Social Worker

Overview
The Social Worker will play a key role in supporting the emotional, social, and psychological well-being of our participants, who may be dealing with chronic illnesses, cognitive impairments, disabilities, or behavioral health needs. This position involves conducting assessments, developing care plans, providing counseling, and coordinating resources to enhance the quality of life for adults and seniors in a community-based setting.

Duties

● Conduct initial psychosocial assessments and periodic reassessments for all participants.

● Develop and implement individualized care plans in collaboration with the interdisciplinary team, addressing social, emotional, and environmental needs.

● Provide short-term counseling, crisis intervention, and supportive services to participants and their families.

● Advocate for participants by connecting them with community resources, including mental health services, housing support, transportation, and financial assistance.

● Facilitate care coordination and discharge planning as needed.

● Maintain accurate, timely documentation in compliance with state, federal, and facility guidelines.

● Serve as a liaison between participants, families, healthcare providers, and community agencies.

Qualifications

● Master’s degree in Social Work (or related field such as Psychology, Gerontology or Counseling) from an accredited program is preferred.

● Bachelor’s degree in Social Work from an accredited program is preferred with at least 2 years experience.

● Fluency in Mandarin is required to effectively communicate with participants and their families.

● Previous experience working with adults, seniors, or individuals with chronic illnesses, disabilities, or cognitive impairments preferred.

● Proficient in electronic health records (EHR) and Microsoft Office Suite.

Job Type: Full-time

Pay: $28.00 - $40.00 per hour

Expected hours: 35 – 40 per week
