Overview
Join our compassionate hospice care team as a Medical Social Worker.
The Hospice Social Worker provides emotional, psychosocial, and practical support to patients and families facing serious illness and end-of-life care. Working as part of the interdisciplinary team (IDG), the Social Worker helps ensure compassionate, holistic, and patient-centered care that reflects the hospice philosophy and meets all applicable standards. In this meaningful role, you will guide and support patients and families as they navigate the challenges of terminal illness, offering understanding, empathy, and advocacy to promote comfort, dignity, and peace throughout the hospice journey. Your compassion and expertise will make a lasting difference in the lives of those we serve.

What you’ll do

· Evaluate the emotional, social, and environmental needs of patients and caregivers related to the terminal diagnosis. Communicate findings and recommendations to the IDG.

· Provide counseling to patients and families to assist in coping with emotional stress, grief, and end-of-life issues. Offer crisis intervention when necessary.

· Identify and educate the team about cultural factors influencing care (e.g., communication, family roles, customs, and traditions).

· Serve as a liaison between patients, families, and community resources; facilitate access to financial assistance, benefits, or community-based programs.

· Educate patients and families regarding advance directives, funeral arrangements, and legal or financial considerations.

· Maintain accurate, timely, and compliant clinical records and progress notes in the EMR.

· Participate in the development and review of individualized care plans. Attend regular IDG meetings to discuss psychosocial aspects of care.

· Support patients and families in understanding the hospice philosophy, patient rights, and community resources.

· Participate in quality assessment and performance improvement (QAPI) activities as requested.

· Perform additional related duties as assigned by the Clinical Supervisor..

Skills
To thrive in this role, you should possess the following skills:

· Master’s Degree in Social Work (MSW) from an accredited program, or Bachelor’s Degree (BSW) with at least one year of experience under supervision of an MSW, per §418.114(b)(3).

· Minimum of one (1) year in healthcare, preferably hospice or palliative care.

· Knowledge of hospice philosophy and end-of-life care practices.

· Excellent communication, organization, and documentation skills.

· Ability to work independently and collaboratively in a team setting.

· Current CPR certification.

· Reliable insured automobile, valid California driver’s license, and acceptable driving record.

Base Pay: $40.00 - $45.00 per hour
