Job Description:
Connected Health Care is seeking an empathetic and skilled Full-Time Hospital Social Worker for a respected healthcare facility in Bishop, CA. As a Social Worker in the hospital setting, you will play a crucial role in providing emotional, psychological, and social support to patients and their families. This position involves collaboration with a multidisciplinary team to deliver holistic care and assist patients with complex healthcare challenges. Key Responsibilities:

• Conduct psychosocial assessments and evaluations for patients and their families.
• Provide counseling and support for patients dealing with medical, emotional, or financial challenges.
• Assist with care coordination and collaborate with medical staff to develop discharge plans and ensure a smooth transition of care.
• Provide referrals to community resources such as housing, mental health services, and financial assistance.
• Support patients and families in navigating complex healthcare systems and advocate for patient rights.
• Offer crisis intervention services to patients experiencing urgent medical, emotional, or situational crises.
• Document all patient interactions and social work interventions in accordance with hospital policies and regulatory requirements.
• Participate in case reviews and care planning meetings with other healthcare professionals.
• Ensure compliance with state and federal regulations related to healthcare services.

Education and Certification Requirements:

• Master's degree in Social Work (MSW) from an accredited university preferred, Bachelors degree required. 
• LCSW (Licensed Clinical Social Worker) required.
• Valid California state licensure as a Clinical Social Worker.
• A minimum of two (2) years of clinical experience in a hospital or healthcare setting is preferred.
• Basic Life Support (BLS) certification preferred.

Preferred Qualifications:

• Experience in hospital-based social work or medical social work.
• Knowledge of community resources and services available in Bishop and surrounding areas.
• Strong interpersonal skills and the ability to collaborate with healthcare teams.
• Crisis intervention experience and the ability to work in high-pressure situations.
• Strong organizational and time-management skills.
