Together in Caring is a professional geriatric care management practice providing high-quality, person-centered care solutions for older adults, individuals living with Alzheimer’s or other dementias, and those with chronic or complex medical conditions.
Founded in 2021 by Lifespan of Greater Rochester, Entrusted Care, and Mark McDermott of Touching Hearts at Home, our team brings decades of combined experience in aging services. We are expanding our compassionate, credentialed team in Rochester and Syracuse to the Capital District region and seeking a dedicated Care Manager to foster relationships and help older adults and families navigate the challenges of aging with dignity, safety, and peace of mind.
Job Overview
The Care Manager plays a vital role in providing comprehensive, professional care management to older adults and their families—while also serving as a visible ambassador of Together in Caring in the Capital District community.
This position combines approximately 75% client care management with 25% outreach and relationship development, helping to ensure continuity of care for current clients and awareness of Together in Caring’s services among referral partners and community organizations.
The Care Manager may be a Licensed Practical Nurse (LPN), or Licensed Social Worker (LMSW/LCSW) who is skilled in care coordination, advocacy, and collaboration.
Responsibilities
- Care Management (75%)
- Develop and implement individualized care plans focused on safety, independence, and quality of life
- Coordinate and manage services across medical, behavioral, and social domains (home care, housing, legal, financial, transportation, etc.)
- Serve as liaison between clients, families, and providers across healthcare and community settings
- Facilitate hospital, rehab, and skilled nursing facility admissions and discharges
- Provide education and advocacy to clients and caregivers regarding available resources and care options
- Conduct regular home visits to monitor progress and adjust care plans as needed
- Offer emotional support and guidance to clients and families during transitions and decision-making
- Maintain accurate documentation of assessments, care plans, and follow-up activities
Outreach & Relationship Development (25%)
- Build and maintain strong referral relationships with professionals such as healthcare providers, senior living communities, elder law attorneys, and home care agencies
- Represent Together in Caring at community meetings, networking events, and educational presentations
- Conduct outreach visits, deliver informational materials, and follow up on leads to generate new referrals
- Collaborate with the marketing and outreach team to identify opportunities for local visibility and engagement
- Track outreach activities and maintain referral database
Qualifications
- Current NYS license as an LPN, LMSW, or LCSW (Required)
- 2+ years of experience in care management, case management, or related field (Required)
- Experience working with older adults or individuals with dementia/chronic illness (Required)
- Strong interpersonal and relationship-building skills
- Excellent verbal, written, and presentation abilities
- Ability to work independently and collaboratively in a hybrid environment
- Valid NYS Driver’s License and reliable transportation (Required)
