Job Description
Renova Health is one of the leading Wellness companies in the US. We have an immediate opening for a Care Manager in Abingdon, VA. As a member of our Healthcare team, you will have the opportunity to help improve the quality of life for many of our Medicare patients. Your primary job responsibility will be to provide telephonic clinical expertise and oversight for Chronic Care Management of patients to discuss care plans and provide patient education. The key tasks for your job will include speaking and interacting with patients over the phone and in person, electronic documentation, scheduling, enrollment with patients in person, and the ability to work well with patients, providers, and internal Renova staff in order to optimize patient care. This job is working with patients on the phone and in person. This is a Hybrid role; you will be working 2 or 3 days remote and 2 or 3 days on-site in Abington.
Essential Job Functions/Responsibilities:
- Make numerous outbound calls per day to patients/providers/offices/others.
- Provide and facilitate in-person enrollment counseling services to potential patients in a fair, impartial manner and in a way that is individually appropriate.
- Constant communications to and from medical provider regarding patient need and/or issues and to update provider on the on-going status of the patient.
- Track and monitor enrollment activities.
- Conduct Advanced Care (Directives) planning along with developing Care Plans for qualified Medicare patients.
- Monitor implementation of the Care plans and progress of patients and send recommendations to physicians for review and adjustments if needed.
- Provide twenty minutes of telephonic clinical expertise and oversight for chronic care management of patients to discuss care plans and provide patient education.
- Documents all communication and coordination of patient contact in electronic documentation system. Assures documentation includes tracking and timestamping to support billing for services provided.
- Establish and maintain positive relationships with all assigned patients and corresponding providers and internal Renova staff.
- Attend training sessions and workshops as necessary.
Skills and Requirements:
- Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) for at least 2 years.
- At least two years of experience of working in a physician’s office, hospital, or similar setting.
- Experience working with Allscripts EMR system.
- Excellent written and verbal communication skills, both in-person and on the phone.
- Excellent attention to detail; able to maintain complete and accurate documentation of all patient related activities.
- Ability to build relationships and quickly develop trust with patients and healthcare professionals.
- Ability to utilize positive problem solving in difficult situations and manage stressful situations (i.e. working with patients who may have severe and persistent health issues)
- Must be organized, detail oriented and the ability to work independently, with minimal supervision.
- Highly motivated, results-oriented with strong skills in customer service, organizing and multi-tasking.
- Ability to educate the patient on independent living skills with attainable increasingly aspirational goals.
- Ability to think clearly and act appropriately in an emergency or potentially dangerous situation.
- Ability to maintain an attitude of objectivity and open-mindedness concerning new approaches and methods.
- Flexible to handle fluctuating workloads and tasks.
