We are looking for a dedicated Hospice Social Worker to join our interdisciplinary team and provide meaningful psychosocial support to patients and families navigating the challenges of terminal illness.
Key Responsibilities
- Conduct initial and ongoing psychosocial assessments for patients and families.
- Develop individualized care plans addressing emotional, social, financial, and practical needs.
- Provide crisis intervention, counseling, and grief support to patients and caregivers.
- Educate families on available community resources such as IHSS, VA benefits, Medi-Cal programs, and caregiver support services.
- Collaborate closely with the interdisciplinary team (RN Case Managers, Physicians, Chaplains, HHAs, and Volunteers) to ensure holistic care.
- Participate in IDG meetings, care plan updates, and family conferences.
- Maintain thorough, timely, and compliant documentation per Medicare and state hospice regulations.
- Support families during bereavement and end-of-life transitions, offering emotional support and follow-up as needed.
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited program required.
- Minimum one year of experience in healthcare, hospice, or related field.
- Strong understanding of grief, loss, and family dynamics in the hospice setting.
- Excellent communication, organization, and documentation skills.
- Valid California driver’s license, reliable transportation, and current auto insurance.

Job Type: Part-time
Pay: $50.00 - $60.00 per hour
Expected hours: 20 – 30 per week
