If yes, how you will contribute to the team:

As the Medical Social Worker, you will implement standards of care for medical social work services in accordance with the Social Work Practice Act. Your role focuses on assessing and supporting patients and families facing terminal illnesses, providing counseling, crisis intervention, and connecting patients with community and organizational resources. Your key responsibilities include:

Must Have:

• Graduate of a bachelor’s or master’s program in social work accredited by the Council on Social Work Education.
• Minimum of one (1) year experience in healthcare; home health and or hospice experience preferred.
• Strong verbal, written, and organizational skills.
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• CPR Certification, Tb test clearance and Physical Exam within one year of hire.

Nice to Have:

• Bilingual skills (English/Spanish) are a plus.
