Job Summary
Holistic Care Home Health is seeking a detail-oriented and organized Intake Coordinator/Scheduler to join our team in a collaborative and fast paced office environment. The person will play a crucial role in handling incoming patient referrals, managing appointments, coordinating schedules, and ensuring that all patients are seen by our nurses in a timely manner. This position requires a strong understanding of local geographic areas and the ability to communicate effectively with both patients and our field staff.

Responsibilities

Manage incoming patient referrals and coordinate patient appointments for various services within the clinic.
Utilize phone systems to communicate with patients regarding scheduling and confirmations.
Maintain accurate medical records and ensure all files are updated in accordance with clinic policies.
Collaborate with medical staff to optimize scheduling efficiency and patient flow.
Assist in the management of office operations, including filing and organizing patient information.
Support the implementation of medical coding practices as needed.
Utilize Data Soft Logic for scheduling and record management.
Provide excellent customer service to patients, addressing inquiries and resolving scheduling conflicts.

Qualifications

Previous experience in a home health or medical office setting is required..
Proficient in using phone systems and office management tools.
Strong organizational skills with attention to detail.
Ability to work effectively in a fast-paced environment while managing multiple tasks.
Excellent communication skills, both verbal and written.
Experience in a home health or hospice setting is beneficial but not required.
Knowledge of Data Soft Logic or similar software systems is an advantage.
