The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

The responsibilities of the BCSC Licensed Mental Health Counselor include, but are not limited to:

Responsibilities:

  • Participates in the behavioral health assessment and examination of BCSC PACE applicants and participants.
  • Serves as a member of the PACE Interdisciplinary Team.
  • Evaluates the needs and participates in the development of a comprehensive behavioral health program.
  • Makes nursing home/or home visits when appropriate and as directed by the BCSC PACE physicians.
  • Participates in all programs of health restoration, prevention, promotion and maintenance of health.
  • Attends and participates in staff meetings.
  • Fulfills all administrative requirements, including statistical records, chart entries, etc.
  • Provide diagnosis, care planning, and behavioral health treatment, including psychotherapy, to treat a wide range of behavioral health concerns.
  • Participate in Interdisciplinary Team (IDT) meetings and the care planning process to identify knowledge gaps around behavioral health issues and educate IDT member around behavioral health assessments, goal setting, and interventions, including mental illnesses, substance use disorders and behavioral manifestations of dementia.
  • Work with clinicians and fellow staff to approve integration of behavioral health interventions into plans of care.
  • Establish relationships with, and ensure effective communication with community behavioral health specialists, and make referrals to community behavioral health specialists, as appropriate.
  • Coordinate with consulting Geriatric Psychiatrists to ensure appropriate exchange of information and utilization of resources.
  • Follow participants admitted to inpatient psychiatric facilities to ensure identified issues are addressed and that follow-up behavioral health interventions are appropriate.
  • Participate in Staff Training on behavioral health topics. May occasionally provide education to staff and participants on behavioral health topics.
  • Other related duties as assigned.

Education and Experience:

  • Licensed Social Worker, State of California.
  • Verification of completion of a CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a health care provider is required for employment.
  • Meet standards of Board of Behavior Health and the appropriate Business and Professional Code, State of California.
  • Successful completion of a program of study, which conforms to Board of Registered Nursing standards.
  • At least one year of experience working with a frail or elderly population
  • Willingness to work within the framework of a multidisciplinary team in the BCSC PACE health delivery system.
  • Experience in geriatrics preferred.
  • Bilingual ability may be required for specific assignments.
  • Must have a valid California driver’s license.
