The Care Coordinator in home health is responsible for ensuring that patients receive high-quality care in their homes by coordinating all aspects of the care plan. This includes organizing medical services, arranging support for patients, and collaborating with healthcare providers to meet patient needs. The Care Coordinator will act as a liaison between patients, their families, medical professionals, and other service providers.

Key Responsibilities:

• Collaborate with healthcare providers to create personalized care plans for patients based on their medical needs, goals, and preferences.
• Monitor patient progress and adjust care plans as necessary.
• Patient & Family Support:
• Provide education and guidance to patients and their families on care plans, procedures, medications, and health conditions.
• Offer emotional support to patients and families, helping them navigate the challenges of home healthcare.
• Case Management:
• Manage patient cases by coordinating visits from medical professionals such as nurses, therapists, and home health aides.
• Ensure that all prescribed care is delivered in a timely manner and that any changes in the patient’s health are communicated promptly.
• Resource Coordination:
• Coordinate additional services such as medical equipment delivery, transportation, and home modifications when needed.
• Ensure that necessary resources, such as medications or home health aids, are available and accessible.
• Documentation & Compliance:
• Maintain accurate and up-to-date patient records, including health status, progress notes, and treatment plans.
• Ensure compliance with state and federal regulations, including patient privacy laws (HIPAA).

Required Qualifications

• Previous experience in Home Health Care

Pay: $20.00 - $21.00 per hour
