Homecare Intake Coordinator

As a Homecare Intake Coordinator at VNA Health Group, The RN in this position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care. The person in this role will be stationed at one of our partner hospitals such as; Clara Maass Medical Center or one of the other major hospitals we partner with in Essex County.
What You’ll Do:
Coordinates referrals to home care to promote continuity of care
Provides individualized counseling to patient and family regarding available home care services
Establishes and maintains professional working relationship with hospital staff, physicians, and VNAHG staff
Provides back up coverage to assure availability of intake coordinator services
Adheres to the organization’s policy in regards to absenteeism and appearance
Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

RN; Bachelor’s degree in Nursing preferred
Valid NJ Registered Nurse license
2+ years of community health nursing experience
Excellent verbal and written communication skills
Excellent interpersonal and organizational skills
Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
Strong decision making skills
Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $76,580 to USD $107,220. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
