Job Summary:
Care Coordinators will be responsible for providing support to clients at one of NLDP's
hotels by helping them make tailored treatment plans that outline their goals,
identify barriers and establish interventions to overcome the barriers.
This role requires a strong understanding of the challenges faced by vulnerable
populations, as well as the ability to navigate complex systems to ensure clients receive
the help they need.
Responsibilities:
Provide case management services to clients, including needs assessments, treatment
planning, goal setting, and service coordination.
Develop and maintain relationships with clients, their families, and healthcare providers
to ensure comprehensive support.
Utilize knowledge of government programs, such as Section 8 housing, to assist clients
in accessing available resources.
Monitor clients' progress toward their goals and adjust their plans as necessary.
Identify and address potential crises, providing or referring for crisis management and
intervention services as needed.
Collaborate with other service professionals to ensure a holistic approach to client care.
Maintain accurate records of client interactions and services provided.
Support other case managers to promote a culture of teamwork and excellence.
Other duties as assigned.
Skills:
Experience working with vulnerable populations, including those with addiction issues -
Strong organizational and time management skills.
Ability to work independently and as part of a team. Proficiency at Microsoft Office, word
and Excel.
Excellent communication and interpersonal skills.
Strong mentoring and supervising skills
Must be able to pass a background check and drug test.
