Crouse’s Care Coordination Services team is hiring a Full Time Social Worker.

Pay Range: $60,000.00 - $66,000.00 / annual

Schedule: Full Time, 8:00am-4:30pm.

Social Worker Position Responsibilities

  • Works collaboratively with the Care Managers and other members of the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.
  • Participates in the development and implementation of a comprehensive integrated discharge plan.
  • Identifies patient and family psychosocial and environmental needs to admission, diagnosis, treatment and discharge.
  • Actively participates in the discharge process following federal and state regulations and accreditation standards managing the care across the continuum to ensure a safe discharge within a timely manner.
  • Provides crisis interventions to patients and families as needed.

Social Worker Position Requirements

  • Master’s Degree in Social Work required.
  • LMSW license preferred.
  • 2 to 3 years Social Work required.
  • Case Management, Discharge Planning or home care experience preferred
  • Excellent oral and written communication and interpersonal skills.
  • Demonstrate age appropriate communication in assigned work area.
  • Ability to interact with physicians, members of the multidisciplinary healthcare team, patients and families and community agencies to achieve optimal patient outcomes.
  • Knowledge of federal and state regulations (DOH, Medicare/Medicare).
  • Strong organizational and problem solving skills.
  • Knowledge of third party payers and /or managed care principles.
