We are actively seeking an experienced Social Worker to join our skilled nursing interdisciplinary team in Lockport, NY. Social worker is responsible for assuring that the medically related, emotional and social needs of our residents are met and maintained in accordance with current federal, state and local standards, established policies and procedures, and as instructed by the Director of Social Work. Competency in care planning, MDS, BMARC, and discharge planning is a plus.

Education

Must possess, as a minimum, a Bachelors Degree in Social Work, or in a related field.

Experience

Must have, as a minimum, a bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology; and

Must have, as a minimum, one (1) year experience in a health care setting working directly with individuals.
