Patient Services Coordinator – Manhattan Office

Concierge Home Care Agency is seeking an experienced Patient Services Coordinator to join our Manhattan team. The ideal candidate will play a key role in managing patient care and ensuring seamless communication between patients, families, caregivers, and staff.
Responsibilities:
- Schedule and coordinate services for clients, families, caregivers, and providers.
- Manage a small caseload with attention to detail and responsiveness.
- Maintain ongoing communication with agency personnel, referral sources, and contracted partners to support daily operations.
- Utilize the HHAExchange scheduling system to manage patient services effectively.
- Participate in additional coordination activities as needed.
Qualifications:
- A high school diploma is required; some college or business education is preferred.
- Minimum 2 years of experience in a Home Care setting.
- Proficiency with HHAExchange is required.
- Bilingual Spanish preferred.
- Excellent phone, organizational, and communication skills are required.

Job Type: Full-time
Pay: $27.00 - $30.00 per hour
Expected hours: 40 per week
