Job Title: Case Manager / Licensed Social Worker
Location: Brockton, MA (with local travel required)

Position Overview:

We are seeking a dedicated and compassionate Case Manager / Licensed Social Worker to join our team in the Brockton, MA region. The Case Manager will be responsible for coordinating care, conducting assessments, and ensuring members receive the support and services they need to maintain their health, independence, and quality of life. This role requires a strong understanding of social work principles, care coordination, and community resources.

Key Responsibilities:

Conduct comprehensive assessments, home visits, and ongoing evaluations of members’ needs.
Develop and maintain individualized care plans in collaboration with members, families, and healthcare providers.
Provide supportive counseling, advocacy, and referrals to appropriate community and social services.
Monitor members’ physical, emotional, and social well-being, ensuring quality and compliance with program requirements.
Educate and support caregivers to strengthen their ability to provide safe and effective care.
Document all visits, interventions, and care coordination activities accurately and in a timely manner.
Work collaboratively with an interdisciplinary team, including nurses, physicians, and other professionals.

Qualifications:

Licensed Social Worker (LSW, LCSW, LICSW) in the state of Massachusetts or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field with relevant experience.
Previous experience in case management, home care, or community-based services strongly preferred.
Knowledge of state regulations and local community resources is a plus.
Strong organizational, communication, and problem-solving skills.
Ability to work independently, manage a caseload, and travel within the Brockton area.
Valid driver’s license and reliable transportation required.

Job Types: Full-time, Contract

Pay: $20.00 - $22.00 per hour
