Seeking a Full Time Medical Receptionist/Care Coordinator to work in BCHP's Pediatric Office in Garrison, NY. Pay will be approx. $20/hr-$24/hr, negotiable based on experience.

Organizational Duties and Responsibilities
- Performs duties to support the goals and objectives of Boston Children's Health Physicians in its -efforts to provide quality service and patient satisfaction
- Demonstrate continuing education by completing all required annual trainings, as stated in the - Training Curriculum, including but not limited to care coordination, population health management and self-management support techniques.
- Complete annual corporate compliance training
- Understand and effectively communicate HIPAA compliance, corporate compliance and client confidentiality
- Ensure and/or remain in compliance with local, state and federal regulation
Essential Tasks, Duties and Responsibilities
- Office responsibilities
- Verify all patient and family demographics
- Verify all insurance information
- Check the online pre-visit registration through the Phreesia program, adding and/or correcting any information in the patient database.
- Follow-up on any positive responses in the Coronavirus screening through Phreesia
- Phreesia should also be checked at the end of the day to prepare for the next day's visits. Incomplete registration and positive screenings should be dealt with at that time.
- Checking the online payment of balances and copayments, and assisting parents in making these payments at the time of the visit if not done prior to the visit
- Assisting new patients in completing the registration forms
- Updating all information in the patient's electronic record, including removing an Alerts that no longer apply
- Assist patients in obtaining access to the Patient Portal for each child in the family
- Manage referrals to specialists and insurance inquiries.
- Make appointments for future visits for patients as they exit.
- When necessary, assist in answering telephone calls, making and changing appointments
- Experience with Electronic Medical Records is essential, and familiarity with the NextGen EMR is a plus
- Use EHR to track all specialists referrals, ensuring patients have scheduled appointments (assisting the patients with scheduling the appointments as needed), patients are seen by the specialist, and that the consultation notes from specialists are received and sent to the primary care provider within the predefined time frame, as noted in policy, and notating all efforts made in the EHR
- Use EHR to track all lab tests, ensuring patients go for their lab tests, and that the lab results are received and sent to the primary care provider within the predefined time frame, as noted in policy, and notating all efforts made in the EHR
- Use EHR to track all imaging tests, ensuring patients have scheduled appointments (assisting the patients with scheduling the appointments as needed), patients are seen for their tests, and that the imaging results received and sent to the primary care provider within the predefined time frame, as noted in policy, and notating all efforts made in the EHR
- Proactively contacting patients who have unplanned ED visits or hospital admissions
- Run population health reports as instructed by supervisor and/or BCHP Quality Dept
- Run care management reports as instructed by supervisor and/or BCHP Quality Dept
- Assist patients in making appointments with specialists
- Assist patients in issues with school placement
- Assist patients in issues with social service needs such as food stamps, Medicaid, and WIC
- Assist providers in making contact with specialists to get updates about patients and care plans
- Assist providers in following up with patients to get updates in between regular office visits
- Track PCMH compliance by regularly running reports to see performance on elements required for PCMH recognition
- Assist office manager and lead doctor in PCMH transformation by giving input for necessary workflow changes and other changes to make PCMH recognition a reality
- Be a supporter of PCMH transformation
- Develop office workflow suggestions that further the efforts towards team based care
- Work with Quality Department on projects related to continuous quality improvement
- Be a subject matter expert on PCMH within the office
- Provide care in a patient-centered manner
- Attend medical home meetings
