About Job

The Care Coordinator 2 will work in collaboration and continuous partnership with individual program participants and their families. The goal is for participants to attain increased continuity of care with the Care Coordinators help in managing relationships with various providers and community partners and increasing participants safety and stability.

  • Provide frequent one-on-one individualized support in home and community with participants.
  • Develop specific, individualized plans that serve as road maps for clients to get them to a self-sufficient status where they are working towards self-reliance.
  • Monitor adherence to individual service plans, evaluate effectiveness, document participants progress in a timely manner, and facilitate changes as needed.
  • Counsel participants regarding plans for meeting needs using Trauma Informed Care with Motivational Interviewing in aiding participants to mobilize inner capabilities and community resources for improving social functioning.
  • Assist and educate participants with connecting to other services such as employment, legal counseling, substance use and behavior health treatment.
  • Advocate on participants behalf when functioning in community and multidisciplinary team meetings.
  • Identify, partner and collaborate with multiple other service areas both within MDC and the community to develop a holistic service package to meet the individual participant needs.
  • Assist participants in securing entitlements such as SSI, AFDC, Food Stamps, Medical Coupons, State General Assistance, Local Welfare, Child Care and Veteran Benefits.
  • Compile accurate data, prepare accurate reports and conduct follow up procedures utilizing an electronic case management reporting data system.
  • Manage a crisis in a calm, supportive and proactive manner.
  • Other duties as assigned


Schedule: Mon - Fri, 8am - 430pm

  • BA in Psychology, Sociology, Social Work, Mental Health or closely related field of study required; MA in Psychology, Sociology, Social Work, Mental Health or closely related field of study preferred
  • Three years' experience working closely with individuals and families in need of assistance; understanding the theory and practice of multiple evidence-based and best practices such as Motivational Interviewing, housing First, Trauma-Informed Care, Recovery Models, Critical Time Intervention, Consumer Involvement, Human Potential and Resiliency, Strength-based Case Management, Harm Reduction and Treatment and Prevention Planning
  • Ability to obtain Counselor Agency Affiliated Registration
  • Any combination of relevant education and experience will be considered.
  • Familiar with HIPAA guidelines.
  • Demonstrated knowledge and ability to access entitlement programs, with proven ability to maintain accurate, thorough, timely and complete case files.
  • Knowledge and ability to successfully implement interviewing, counselling methods, principles, and techniques that result in measurable outcomes aligned with the agency Theory of Change
  • Ability to analyze interventions for effectiveness, research and develop alternative interventions, openly engage supervisory guidance and input
  • Knowledge of electronic recordkeeping for client information
  • Washington State Drivers License may be required to drive company vehicle; must be insurable-for purpose of transporting clients to scheduled appointments and/or visiting and monitoring clients living environment
