Overview
The Medical Social Worker plays a vital role in providing support to patients and their families as they navigate the complexities of healthcare. This position involves assessing patients' needs, developing care plans, and coordinating resources to ensure comprehensive patient care. The Medical Social Worker will work collaboratively with healthcare teams to address psychosocial issues and promote overall well-being.
Duties
- Conduct comprehensive patient assessments to identify psychosocial needs and barriers to care.
- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare providers.
- Provide counseling services including individual, group, and family therapy to support mental health and emotional well-being.
- Facilitate crisis intervention and management for patients experiencing acute distress or emergencies.
- Coordinate with community resources and services to assist patients in accessing necessary support systems.
- Maintain accurate medical documentation in compliance with HIPAA regulations and organizational policies.
- Participate in multidisciplinary team meetings to discuss patient progress and adjust care plans as needed.
- Conduct intake assessments for new patients, ensuring all relevant information is gathered for effective treatment planning.
- Engage in clinical research activities to contribute to the advancement of social work practices within the medical field.
Skills
- Proficient in child & family counseling, addiction counseling, and behavioral therapy techniques.
- Strong knowledge of medical terminology, medical records management, and EMR systems.
- Experience in hospice care, geriatrics, pediatrics, and working with individuals with developmental disabilities or autism.
- Familiarity with diagnostic evaluation methods including ICD-9 and ICD-10 coding.
- Ability to perform case management duties effectively while demonstrating excellent organizational skills.
- Skilled in motivational interviewing techniques and psychotherapy modalities such as cognitive behavioral therapy.
- Knowledge of crisis intervention strategies and home & community care practices.
- Experience working within inpatient settings, nursing homes, outpatient clinics, or assisted living facilities.
- Understanding of special education needs and dementia care approaches. Pay: $100.00 - $110.00 per hour
Expected hours: No less than 35 per week
