Care Coordinator / Facilitator – Wraparound Program
Full-Time | $68,640 annually | Sacramento, Woodland, and Napa Areas (travel required)
Are you passionate about supporting families and creating environments where children and youth feel valued and understood? Parent Cooperative Community (PCC) is seeking a Care Coordinator / Facilitator to join our Wraparound team. This role is at the heart of helping families thrive—leading Child and Family Team (CFT) meetings, developing individualized plans of care, and ensuring every family receives hope-filled, strength-based support.

What You’ll Do
- Facilitate Child and Family Team (CFT) meetings, bringing families and support systems together.
- Develop and monitor individualized Plans of Care (POC) that reflect family priorities and goals.
- Partner with families to identify natural supports and community resources.
- Mediate conflicts and promote collaborative problem-solving within CFTs.
- Provide crisis support, guiding families through safety planning and emergency coordination.
- Track progress and outcomes using tools such as the Child and Adolescent Needs and Strengths (CANS) assessment.
- Supervise and mentor 3–5 Family Support Specialists, ensuring quality and fidelity to Wraparound principles.
- Work as part of an interdisciplinary team with the Wraparound Supervisor and Therapeutic Support Specialists.

- Ability to collaborate with diverse teams and communities with cultural humility and inclusivity.
- Bachelor’s degree in social work, psychology, counseling, or related field (preferred).
- Experience working with children, families, and/or multidisciplinary teams (preferred).
- Valid driver’s license, reliable transportation, and willingness to travel.
- CPR/First Aid certification a plus.
- Knowledge of trauma-informed care and facilitation skills a plus.

Schedule & Travel
- Monday–Friday, 8:00 AM – 5:00 PM, with some evening/weekend on-call availability.
- Travel to family homes, schools, and community agencies is required.
