Position Overview:

We are seeking a compassionate and qualified Masters-level Social Worker (MSW) for an independent contractor position to join our team for a permanent hire. The role focuses on providing psychosocial support, case management, and discharge planning services in a community-based healthcare setting.

Key Responsibilities:

• Provide counseling, support, and resource referrals to patients and families
• Collaborate with a multidisciplinary team to develop and implement care plans
• Conduct psychosocial assessments and crisis intervention as needed
• Assist with discharge planning and coordination of community services
• Maintain accurate documentation in accordance with clinic policies

Requirements:

• Master’s Degree in Social Work from an accredited institution of Social work – Required
• Valid California Driver’s License and access to an insured vehicle – Required
• Bilingual (English/Arabic) – Highly Preferred
• Strong communication, documentation, and interpersonal skills
