We are a community-based adult day health center providing care and support for adults with medical and psychosocial needs . Our program offers medical, therapeutic, rehabilitative, and recreational programs and services with the goal of preventing premature institutionalization by offering a stimulating and enjoyable experience for the participants.

Job Description

We are seeking a Chinese-speaking Social Worker/Case Manager responsible for person-centered care planning of clients with health concerns. Work days are Mon - Fri.

Responsibilities

• Provide initial and ongoing physical and psychosocial assessments
• Develop and implement a comprehensive plan of care based on the participant’s needs and goals
• Serve as a liaison with the participant’s home and family, and with other community agencies that may be providing services to the participant to meet their needs.
• Serve as a resource for available services
• Coordinate individual and group counseling as defined in individual plan of care
• Monitor and report case recordings on a timely basis
• Assist in daily management of the adult day program
• Have knowledge of ADHC/CBAS laws, regulations, CBAS certification standards of participation and policies and procedures. This will be ensured through ongoing staff training and education.

Required Qualifications:

• Bachelor's degree in Social Work, or Master's Degree in Social Work, Psychology, Gerontology, or Counselling.
• People-oriented with experience in ADHC/CBAS, social work, gerontology or health-related services.
• Strong oral and written English with ability to speak and write Chinese.
• Proficient in Office 365 and able to quickly learn our industry software application.
• Valid driver's license with ability to conduct home visits or drive company vehicle when necessary.

Job Type: Full-time

Pay: $22.00 - $24.00 per hour
