Job Summary:
Under the general direction of the Director of Health & Wellness, the Social Services Coordinator provides comprehensive direct services to individuals and families within Ela Township. This position specializes in hands-on case management, General Assistance (GA) and Emergency Assistance (EA) support, community outreach, and resource navigation. The Social Services Coordinator serves as a dedicated advocate and navigator, helping clients access essential services including housing, mental health resources, financial assistance, and social services. This position requires extensive knowledge of local and state-wide services available to moderate and low-income populations and the ability to build meaningful relationships with both clients and community partners.
Normal business hours are Monday to Thursday, 8 AM to 5 PM, and Friday, 8 AM to 1 PM. Some evening and weekend work may be required for group programming and community presentations.
Key Responsibilities:
Direct Client Services
- Maintain a caseload of 20-35 active clients, primarily serving adults 18 years and older
- Conduct comprehensive assessments to determine client needs, strengths, and barriers to accessing services
- Provide hands-on assistance with completing applications for SNAP benefits, GA/EA forms, housing applications, and other social service programs
- Develop individualized service plans in collaboration with clients, ensuring appropriate support services are identified
- Conduct regular follow-up meetings with clients (ranging from one-time consultations to weekly meetings until goals are achieved)
- Provide direct support and advocacy to help clients navigate complex social service systems
- Maintain accurate, timely, and thorough case documentation in TherapyNotes and VisualGA systems
- Monitor client progress and adjust service plans as needed to promote self-sufficiency
GA/EA Administration
- Conduct intake assessments for Township General Assistance and Emergency Assistance programs
- Assist clients with completing GA/EA applications within required 30-day timeline
- Gather and review all necessary documentation and information pertinent to each case
- Present completed applications to Township Supervisor for eligibility determination
- Ensure compliance with all GA/EA guidelines and maintain detailed case files
- Track application deadlines and follow up with clients as needed
Community Outreach & Resource Development
- Establish and maintain relationships with local service providers, social service agencies, and community organizations
- Actively identify and develop new referral sources to expand available resources for clients
- Collaborate with multidisciplinary teams to coordinate comprehensive care
- Facilitate group sessions and educational workshops on topics such as benefits navigation, financial literacy, and resource awareness
- Develop and deliver presentations to community groups, schools, and service providers
- Represent the Township at community meetings and networking events
- Maintain current knowledge of available local, state, and federal programs and services
Basic Qualifications:
- Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Healthcare, or related field from an accredited institution
- Minimum 5 years of experience in social services, mental health, healthcare, or related field providing direct client support
- Experience working with low-income populations and knowledge of social welfare systems
- Proficiency with case management software and database systems (TherapyNotes and VisualGA preferred)
- Strong computer skills including Microsoft Office Suite (Outlook, Word, Excel, PowerPoint, OneDrive)
- Excellent interpersonal, verbal, and written communication skills
- Demonstrated ability to work with diverse populations, including individuals with behavioral, cognitive, and learning challenges
- Strong organizational skills and attention to detail, particularly regarding deadlines and documentation requirements
- Ability to work independently while maintaining professional relationships with staff, clients, and external agencies
- Valid driver's license and reliable transportation for community visits
- Must pass employment background check and drug testing
- Serves as Mandated Reporter for child abuse, neglect, and elder abuse
Preferred Qualifications:
- Relevant professional certification (CCM, CCMC, LSW, LPC, or other social services/mental health credentials)
- 3+ years of experience in case management or social services
- Experience working within local government (township, county, or municipal) environment
- Experience with DCFS, DHS, or other state social service systems
- Bilingual capabilities (Spanish preferred)
- Experience facilitating group programming and community presentations
- Knowledge of HIPAA regulations and confidentiality requirements
Work Environment / Physical Requirements:
Work is performed in both office and community settings. Regular travel throughout the community for client visits and meetings with partner organizations is required. May be required to work outside normal business hours for group programming and community events. Physical demands include occasional standing, walking, lifting/moving objects up to 30 pounds, and operation of a motor vehicle. Pay: $48,000.00 - $62,000.00 per year
