Responsibilities:

• Manage a case load of 10,000–15,000 patients with identified gaps in care in collaboration with the healthcare team.
• Work with the Patient-Centered Medical Home (PCMH) Team to design, implement, and evaluate the PCMH model as applicable.
• Proactively coordinate preventative and follow-up care for all patients receiving care from participating providers.
• Review patient medical records to identify care gaps and schedule necessary preventive services using evidence-based, physician-approved protocols.
• Document all patient encounters accurately and timely.

Requirenments:

• Updated Resume
• BLS
• LVN License
• Must reside in Texas
• Minimum 2 years of experience in a primary care clinic

Job Type: Contract

Pay: $35.00 - $39.00 per hour
