Job Title: LTSS Service Care Manager – LVN/LPN
Location: Laredo, TX (Hybrid with local travel for F2F visits)
Duration: 6 months contract (possibility of extension)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM CST (1-hour lunch)
Pay Rate: Up to $24.00 - $26.00/hour on w2.
Client: Centene – Superior Health

About the Role

The LTSS (Long-Term Services & Supports) Service Care Manager – LVN plays a critical role in supporting members through comprehensive care coordination. This position focuses on conducting assessments, developing service care plans, and coordinating with interdisciplinary teams to ensure quality, cost-effective outcomes for long-term care members.

Key Responsibilities

  • Conduct telehealth biannual assessments and in-person visits as needed in the Laredo area.
  • Evaluate members’ needs, available resources, and recommend/coordinate appropriate care plans.
  • Develop, monitor, and update long-term service care plans in collaboration with providers, specialists, and community resources.
  • Coordinate between members, families/caregivers, and healthcare providers to ensure accessibility of services.
  • Provide education to members and caregivers regarding benefits, available services, and care instructions.
  • Document and maintain care management activities to ensure compliance with state, federal, and payer regulations.
  • Participate in staff meetings, team huddles, and 1:1 sessions; collaborate with multi-disciplinary teams.
  • Ensure strict turnaround times for documentation of assigned assessments and tasks.
  • Identify opportunities for improvement and provide feedback to leadership.

Qualifications

Required:

  • Active LVN License in Texas
  • Minimum 2 years of LVN experience
  • Located in Laredo, TX with ability to travel locally for face-to-face visits
  • Proficiency in Microsoft Word, Outlook, Excel, and Teams
  • Strong organizational and time management skills

Preferred:

  • Bilingual (English/Spanish)
  • Experience in community health, home health, or care coordination
