About Us:
Citrus Heights Terrace is a specialized memory care community managed by CiminoCare, a family-owned company dedicated to "Old World Hospitality" and compassionate, person-centered care. We are seeking a Resident Care Coordinator with experience in Skilled Nursing Facility (SNF) social services or discharge planning to join our leadership team.

Position Overview:
As the Memory Care Resident Care Coordinator, you'll play a critical role in coordinating care and advocating for our residents living with dementia or Alzheimer's. You'll ensure quality care, collaborate with families and health professionals, and help create a warm, supportive environment.

Responsibilities:

• Coordinate and monitor individualized care plans
  
• Support resident transitions and discharge processes
  
• Serve as the primary point of contact for families
  
• Lead care conferences and collaborate with clinical teams
  
• Assist with behavioral support and emotional needs
  
• Maintain accurate documentation and compliance

Qualifications:

• Experience as a Social Services Coordinator or Discharge Planner in a Skilled Nursing Facility (SNF)
  
• Knowledge of memory care practices and dementia-related behaviors
  
• Strong communication and organizational skills
  
• Compassionate, proactive, and resident-centered mindset

Schedule & Compensation:

• Full-Time: 8-hour shifts, Sunday through Thursday
  
• Salaried position with competitive pay
