Our Community Residence has one opening for a Service Plan Writer (Day Time Residence Counselor). This is a full time position and the hours are Monday – Friday 8AM-4:30PM (hours are somewhat flexible). Minimum qualifications for Service Writer: High School diploma or equivalent and one year experience in the Human Services field. Valid NYS driver's license is required.

Service Writer responsibilities include collaborating with staff and clients on developing service plans according to OMH and Medicaid regulations. The Service Writer will also review weekly contact notes, perform quarterly chart audits, and complete documentation according to agency standards. The ability to type, and to write clearly and objectively is a must. When down staffed, coverage of clients is required.
