Eliot is seeking a dedicated bilingual(Spanish) Care Manager to join our Behavioral Health Community Partner (BH CP) program. This innovative Care Management program serves members with complex medical, behavioral health, and social needs. As a Care Manager, you will work with a multidisciplinary team to conduct comprehensive assessments and develop treatment plans for clients. In this role, you will build relationships with clients, medical and health providers, family and other service providers. This position requires highly motivated and organized individuals capable of managing a caseload of clients.


Responsibilities:

  • Promote hope and empowerment through support and meetings with clients
  • Empower clients to exercise autonomy in all aspects of their lives
  • Help clients identify choices and develop self-advocacy skills
  • Provide interventions focused on problem-solving, skills training, behavior modeling, optimism, and encouragement
  • Support the team and clients in the ongoing assessment of symptoms and responses to interventions
  • Ensure that all required assessments are completed in a timely manner in compliance with MassHealth and contract guidelines
  • Contribute to the development of assessments and treatment plans for each client.
  • Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and focused on recovery and clients' wellness goals
  • Engage clients in the treatment planning process, ensuring their preferences are embraced and collaborated with the enrollee's PCP and Care Team
  • Ensure that enrollees are treated with dignity and respect in accordance with ECHS' policy


Qualifications:

  • Requires a Bachelor's degree in Psychology, Social Work or related field
  • Experience working with adults with mental illness/substance abuse
  • Valid Massachusetts drivers license
  • Fluent in Spanish and English


Schedule:

  • Monday-Friday, 9am-5pm
