Duties Description As an Intensive Case Manager, your duties will include:
- Assessing individuals, developing individual care plans, and determining necessary resources
- Advocating for and assisting individuals in accessing and obtaining services
- Contacting service providers to determine appropriate placements and referrals
- Monitoring individual rehabilitation goals and progress
- Providing crisis intervention services
Minimum Qualifications:
Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as an Intensive Case Manager if they have:
- Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, or speech and hearing;
- 4 years of experience providing direct services to individuals, 50% of whom have been diagnosed with mental illness, developmental disabilities, and/or substance use disorder or 4 years of experience linking individuals, 50% of whom have been diagnosed with mental illness, developmental disabilities, and/or substance use disorder, to a broad range of services essential to living successfully in a community. 1 of the 4 years must have included working with individuals, 50% of whom have a primary diagnosis of mental illness.
OR
Candidates from outside or within State Service can be considered for a competitive class appointment if they are reachable on the current Intensive Case Manager eligible list.
OR
Candidates within State Service can be considered for a competitive class appointment if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
Must have a valid NYS Drivers License.
