Job Summary:

The Permanent Supportive Housing Case Manager is responsible for receiving new participant referrals from the Coordinated Entry System, assessing program eligibility, assisting referrals with the necessary documentation, facilitating their transition into housing, and providing ongoing support to residents. The Case Manager also helps residents access community resources, set and achieve personal goals, and promote their overall personal growth and self-sufficiency.

Job Responsibilities:

1. Meet in person at the office with new referrals to verify eligibility, collect income and required documentation, determine rent and location needs, and assist with securing housing.

2. Collaborate with the Housing Specialist, both on-site and off-site, to ensure eligible participants successfully move into housing.

3. Conduct regular meetings, in person or by phone, with participants to assess needs, develop goals, and create action plans.

4. Maintain bi-monthly check-ins with participants to assess progress, update action plans, monitor income, document case notes, and connect clients to resources.

5. Provide ongoing life skills coaching, addressing problem-solving and skill development to support participants’ independence and improved quality of life.

6. Conduct periodic visits to scattered-site apartments, ensuring compliance with lease and program rules, in partnership with property management when necessary.

7. Deliver targeted interventions for participants at risk of eviction, either at the office or at scattered-site apartments.

8. Advocate on behalf of participants to connect them with medical, mental health, employment, and other supportive services.

9. Assist participants with applications, qualifications, and re-determinations for SSI, SSDI, Medicaid, Medicare, SNAP, and other benefits as needed.

10. Distribute grant-funded or donated goods to clients, maintaining documentation and following grant guidelines.

11. Maintain accurate and up-to-date client files, both physical and in the HMIS database.

12. Complete all necessary documentation for program responsibilities or third-party billing.

13. Track and report participant outcomes and provide data updates as directed.

14. Participate in trainings and meetings both on-site and off-site.

15. Attend internal and external case staffing meetings to coordinate services.

16. Perform other duties as assigned.

Qualifications:

1. Education: A Bachelor’s degree in social services or a related field is required.

2. Experience: Substance abuse and mental health experience preferred. Experience working with homeless and/or low-income populations preferred. Several years of case management experience is preferred.

3. Qualifications and experience may include lived expertise, and applicants with such experience are strongly encouraged to apply.

4. Language: English required; bilingual (English and Spanish) strongly preferred.

Other Responsibilities

1. Strong case management skills with demonstrated written and oral communication abilities.

2. Ability to work collaboratively as part of a team.

3. Proficiency in engagement, assessment, planning, and advocacy.

4. Good judgment and ability to work independently.

5. Experience working with individuals with severe disabilities (physical, developmental, mental health, substance abuse, etc.) and a history of homelessness.

6. Strong organizational skills to manage multiple responsibilities effectively.

7. Access to personal transportation is required.

8. Willingness to work occasional nights or weekends.

9. Ability to lift and move up to 50 lbs.

Pay: $24.00 - $26.00 per hour
