Overview
We are seeking a dedicated and compassionate MSW- Bilingual in Spanish to join our team. This role is essential in providing high-quality social work services to our diverse client population. The ideal candidate will possess a strong understanding of case management, patient care, and therapeutic practices, specifically tailored for HIV+, LQBTQIA+ individuals and groups. Your ability to communicate effectively in both English and Spanish will enhance our outreach and support for the community we serve.

Duties

• Provide social work case management services to the residents of the Residential Care Facilities for the Chronically Ill and Transitional Residential Care Facilities programs.
• Participate in the screening of clients to assess for appropriateness of placement.
• Complete initial psychosocial assessment on all new clients prior to admission.
• Participate in case conferences and developing a client services plan with the multidisciplinary team.
• Meet with each client as needed at a minimum of once monthly with a formal re-assessment quarterly.
• Create activities to develop Independent Living Skills.
• Participate in linking residents to services such as case management, housing, transportation assistance, medical care, benefits determination, vocational development or employment placement.
• Participate in monthly case conferences.
• Assist the resident in preparing to move into independent living.
• Assist resident in learning self-sufficiency according to the requirements of DHSP.

Skills

• Proficient understanding of HIPAA regulations
• Strong case management skills with the ability to navigate complex client situations.
• Experience in social work practices
• Knowledge of therapeutic techniques applicable to HIV+ and diverse populations.
• Ability to provide compassionate patient care while maintaining professional boundaries.
• Proficient in conducting therapy sessions that are culturally sensitive and appropriate for bilingual clients.
• Excellent communication skills in both Spanish and English, facilitating effective interactions with clients from various backgrounds.

Mandatory Requirements:

• Master’s in Social Work (MSW) Degree from an accredited college or university.
• Driver’s License and Auto Insurance
• Ability to travel L.A. county-wide
• Ability to pass Live Scan Clearance
• Health Screening Clearance including TB annually

Desired Qualifications:

• Bilingual English/Spanish
• Working knowledge of HIV/AIDS
