Job Description Summary
Bonum Home Health seeks a dedicated Licensed Vocational Nurse (LVN) Care Coordinator to join our team in Burbank, CA. The LVN Care Coordinator plays a critical role in ensuring seamless delivery of home health services by coordinating patient care, maintaining accurate and compliant patient charts, scheduling clinician visits, and facilitating communication with referring physicians, patients, and families. Utilizing Kinnser platform as our (EMR) system, the LVN Care Coordinator ensures compliance with state, federal, and Joint Commission regulations while delivering patient-centered care in a fast-paced home health environment.
Essential Job Functions/Responsibilities
Patient Care Coordination
- Visit Scheduling: Coordinate and schedule clinician visits, including Registered Nurses (RNs), Physical Therapists (PTs), Occupational Therapists (OTs), Certified Home Health Aides (CHHAs), and Medical Social Workers (MSWs), based on patient care plans. Plot visit schedules in the EMR system, ensuring alignment with clinical needs and availability.
- Physician Communication: Act as the primary liaison with referring physicians to communicate clinical needs, request new orders, or obtain order changes. Document all communications in the EMR within 24 hours to ensure continuity of care and compliance.
- Hospitalization Follow-Up: Monitor and follow up on hospitalized patients, coordinating with hospital discharge planners, referring physicians, and the clinical team to update care plans and resume home health services. Record hospitalization status and outcomes in the EMR.
- Order Management: Ensure all physician orders are completed accurately and promptly. Track order status in Kinser, follow up on pending orders within 48 hours, and verify compliance with regulatory requirements.
Patient Chart Management
- Chart Maintenance: Maintain accurate, complete, and up-to-date patient charts in the EMR system, including clinical notes, care plans, physician orders, and interdisciplinary team documentation. Conduct weekly audits to ensure compliance with state, federal, and Joint Commission standards.
- Documentation Oversight: Review clinician visit notes and assessments for accuracy and completeness, flagging discrepancies for correction within 48 hours. Ensure all required documentation is uploaded to the EMR.
Compliance and Recordkeeping
- Regulatory Compliance: Ensure all patient records and clinical documentation comply with HIPAA, state, federal, and Joint Commission regulations. Prepare charts and reports for internal audits and regulatory inspections.
- Reporting: Generate and submit reports in the EMR (e.g., visit compliance, hospitalization trends) to the DON or Clinical Supervisor for internal and regulatory purposes.
Communication and Collaboration
- Interdisciplinary Team Support: Facilitate communication among clinicians, patients, families, and referring physicians to ensure alignment of care plans and timely resolution of clinical issues. Provide updates to the interdisciplinary team (IDG) during care planning meetings.
- Patient and Family Liaison: Address inquiries from patients and families, particularly Spanish-speaking individuals, regarding care schedules, services, or clinical updates. Escalate complex issues to the RN Case Manager or DON as needed.
- Bilingual Support: Provide verbal and written Spanish-language support to patients and families, ensuring clear communication of care plans, visit schedules, and clinical instructions.
Operational Support
- Care Plan Updates: Assist the DON or Clinical Supervisor in updating patient care plans based on physician orders, hospitalization outcomes, or clinical assessments. Ensure updates are documented in the EMR and communicated to the clinical team.
Confidentiality
- Data Protection: Maintain strict confidentiality of all patient, clinician, and organizational information in accordance with HIPAA and Bonum Home Health policies. Ensure secure handling of data in the EMR system.
Position Qualifications
- Licensure: Current and active Licensed Vocational Nurse (LVN) license in the state of California.
- Experience: Minimum of one year of experience in home health care coordination; experience in hospice or similar settings is preferred.
- Language Proficiency: Fluent in Spanish (verbal and written) to effectively communicate with Spanish-speaking patients and families.
- Technical Skills: Proficient in Microsoft Office Suite (Word, Excel, Outlook) and able to quickly learn and navigate Kinser systems for scheduling, documentation, and reporting.
- Organizational Skills: Exceptional organizational skills and attention to detail to manage complex schedules, patient charts, and multiple priorities in a fast-paced environment.
- Communication Skills: Strong verbal and written communication skills in both English and Spanish, with the ability to interact professionally with patients, families, clinicians, and referring physicians.
- Problem-Solving: Ability to anticipate and resolve scheduling or clinical coordination issues proactively to ensure continuity of care.
Job Type: Full-time
Pay: $30.00 - $40.00 per hour
